Who can request a certified copy of a birth or death certificate?

All applicants must present a personal valid ID and complete an application. Qualified Applicants are listed below. 

For Birth Certificates

  • The certificate holder
  • The spouse of the certificate holder, with a copy of the marriage license to prove relationship
  • The parent of the certificate holder, who is listed as such on the certificate
  • The child of the certificate holder, must present a copy of his or her birth certificate with the parent listed 
  • The grandparent of the certificate holder, with a copy of the birth certificate of his or her child who is listed as the parent on the requested certificate
  • A sibling of the certificate holder, with a copy of his or her birth certificate having at least one of the same parents listed
  • Legal Guardian of the certificate holder with a certified court order establishing legal custody 
  • A person presenting legal documentation proving a tangible interest in the certificate

For Death Certificates

  • The spouse of the certificate holder, who is listed as such on the certificate
  • The parent of the certificate holder, who is listed as such on the certificate
  • The child of the certificate holder with a copy of their birth certificate showing the decedent as their parent
  • The grandparent of the certificate holder, with a copy of the birth certificate of their child who is listed as the parent on the requested certificate
  • A sibling of the certificate holder, with a copy of their birth certificate having at least one of the same parents listed
  • A person presenting legal documentation proving a tangible interest in the certificate
  • Attorneys with Valid Bar Card or License, using a letter of request on company letterhead outlining the purpose of the requested document, or a court order or subpoena stating the need for the record
